This Privacy Policy explains how Laforma Hospital (“Laforma”, “we”, “us” or “our”) collects, uses, stores and shares personal information when you visit laformahospital.com, submit an enquiry or consultation request, communicate with us, or otherwise use our website and services.

1. Who we are

Laforma Hospital is a healthcare provider based in Lahore, Pakistan. This website provides information about our hospital, specialists and services and allows visitors to request contact or a consultation. Website information does not replace medical assessment and should not be used for emergencies.

Depending on how you use the website, we may collect:

  • Identity and contact information, such as your name, phone number, email address and preferred contact method.
  • Enquiry and appointment information, such as the treatment or concern you select, your preferred consultation date or contact time, and any additional information you choose to provide.

Image upload is not currently enabled on our public consultation forms. If this feature is introduced, this policy and the relevant form notice will be updated before images are collected.

Information about a treatment interest or health concern may be sensitive. We use this information to respond to your request, support consultation planning and route the enquiry appropriately. Access should be limited to personnel and service providers who need it for those purposes. Please do not include unnecessary medical details in a general website enquiry. For urgent or emergency care, contact the appropriate emergency service or attend an emergency facility rather than using an online form.

We do not intentionally include the treatment selections or free-text health information entered in our forms as parameters in that conversion event.

6. Shopify and other service providers

We may also use service providers for website hosting, security, analytics, advertising measurement, communications, email delivery, professional advice and operational support. They may process information on our behalf or under their own applicable terms, subject to the role they perform.
